If the ball of your foot feels bruised, hot or like there’s a pebble under it — especially when you walk barefoot or wear hard-soled shoes — you may have metatarsalgia. The good news: ball-of-foot pain responds well to offloading the pressure, which is exactly what the right support is designed to do.
What metatarsalgia is
Metatarsalgia is pain and inflammation in the ball of the foot — the padded area just behind your toes. Five long bones, the metatarsals, end here in the metatarsal heads, and they take a lot of your body weight every time you push off a step. When too much pressure concentrates on one or more of those heads, the tissue underneath gets irritated and sore.
Signs & symptoms
- A burning, aching or sharp pain in the ball of the foot
- A feeling like there’s a stone or fold in your sock under the foot
- Pain that worsens when you stand, walk or push off, and eases when you rest
- Tingling or numbness spreading towards the toes
- Pain that’s worse barefoot or in thin, hard or high-heeled shoes
- A callus building up under the ball of the foot
Why it happens
It usually comes down to load. High heels and tight, pointed shoes tip your weight forward onto the ball of the foot. A high arch, a long second toe, bunions or clawed toes can also concentrate pressure on particular metatarsal heads. Add in long hours on hard floors, extra body weight, or the natural thinning of the foot’s fat-pad with age, and the tissue under the ball simply takes more punishment than it can absorb.
How custom insoles help
Custom orthopaedic insoles ease metatarsalgia by changing where your weight lands:
- Metatarsal pad — placed just behind the metatarsal heads, it lifts and spreads the long bones so pressure no longer piles onto the sore spot.
- Cushioning — shock-absorbing material under the ball softens each step and protects thinning fat-pads.
- Offloading — the insole is shaped to redistribute load away from the tender metatarsal head and across the whole foot.
- Arch support — supporting the arch stops the foot collapsing forward, easing pressure on the forefoot.
The aim is relief and support — taking the load off the ball of your foot so the irritated tissue can settle — not a medical cure.
Supportive footwear matters too
An insole can only do so much inside a shoe that crushes your forefoot. A wide toe box, a low stable heel and a cushioned sole let your toes spread and keep your weight off the ball of the foot. Our orthopaedic footwear is built to pair with custom insoles so the offloading carries through every step.
Our fitting process
We start by finding exactly where the pressure peaks under your foot — your arch type, gait and the precise sore spots.
In Pune? Your first fitting is free at our Budhwar Peth shop, where we assess and fit you hands-on.
Anywhere else in India? Our online fitting (₹499, fully credited to your insole order) assesses your feet over video and from a few photos, then we post your custom insoles to your door.
Related conditions
Forefoot pain often travels with other issues. If you also feel burning or tingling between the toes, read about Morton’s neuroma; if your big-toe joint is the problem, see bunions.
When to see a doctor
We’re a specialist footwear shop, not a medical clinic. If your ball-of-foot pain is severe, persistent, follows an injury, or comes with numbness, spreading swelling or signs of infection, please see a doctor for diagnosis first. Our role is to support and offload your feet alongside any medical care you receive.
Frequently asked questions
What is the fastest way to relieve ball-of-foot pain?
Take the pressure off the spot that hurts. A metatarsal pad sits just behind the ball of the foot and spreads load away from the sore metatarsal heads, while cushioning softens the impact. Custom insoles place that support exactly where your feet need it, which is why they tend to feel better than generic pads.
Do metatarsal pads go under the painful area?
Just behind it, not directly on it. The pad lifts and spreads the long bones of the foot so the pressure no longer concentrates under the ball. Placement is fiddly to get right by hand, so we position it precisely on your custom insole during the fitting.
Which shoes are best for metatarsalgia?
Look for a wide toe box, a low stable heel and a cushioned, rocker-style sole that eases the push-off. High heels and tight, pointed shoes push your weight onto the ball of the foot and tend to make things worse. Our orthopaedic footwear is built to work with your insoles.
Can being overweight cause ball-of-foot pain?
Extra body weight increases the load passing through the ball of the foot, so it can add to the strain. Spreading pressure with the right insoles and footwear helps, but if the pain is persistent it is worth a doctor’s review alongside the support we provide.
Do I have to come to Pune for a fitting?
No. If you are in Pune your first fitting is free at our Budhwar Peth shop. Anywhere else in India, our online fitting (₹499, fully credited to your insole order) assesses your feet over video and we post your custom insoles to your door.
